The district health authorities have served show-cause notices on 11 ultrasound centres on finding discrepancies in Form-F, mandatory to be filled in under the PNDT Act to maintain medical records of pregnant women.

Sources said during scrutiny, more than 100 forms were found wherein patients’ home address did not match the address given in Aadhaar card, referral slip was missing, radiologist who performed the ultrasound was not mentioned or identification proof of woman was not attached.

The authorities directed the ultrasound centres to file their replies within the next three days. The district has 32 ultrasound centres and those which have received the notices are located in Jhajjar, Bahadurgarh and Beri.

“Show-cause notices have been issued under Section 20(1) of the PNDT Act as deficiencies are in contravention of rules and are serious in nature,” said Dr Anchal Tripathi, nodal officer (PNDT).

He said every ultrasound centre was bound to fill in all columns of Form-F and attach identity proof and referral slip of the woman before carrying out a test.
